Mesleki hiyerar\u015fi ve multidisipliner \u00f6\u011frenme t\u0131pta bilginin nesilden nesile uygulamal\u0131 aktar\u0131m\u0131n\u0131 sa\u011fl\u0131yor olsa da yasal d\u00fczenlemelerdeki yetersizlikler bu yap\u0131lanman\u0131n suistimaline ve dolay\u0131s\u0131yla sa\u011fl\u0131kta \u015fiddete kap\u0131 aral\u0131yor. \u00dcstelik bu \u015fiddetin d\u00fczeyi ki\u015filerin kimliklerine, k\u00f6kenlerine ve hastane d\u0131\u015f\u0131ndaki hayat tarzlar\u0131na ba\u011fl\u0131 olarak de\u011fi\u015fkenlik g\u00f6sterebiliyor.<\/p>\n\n\n\n<p>Mobbingin tan\u0131m\u0131, \u00fclkeler \u00f6zelinde ve sosyok\u00fclt\u00fcrel ba\u011flama g\u00f6re yap\u0131lmaktad\u0131r. T\u00fcrkiye\u2019de mobbing, i\u015fyerinde mobbing ve hastanede mobbing gibi grupland\u0131rmalara gitmek ve kavram anlay\u0131\u015f\u0131n\u0131 spesifikle\u015ftirmek m\u00fcmk\u00fcnd\u00fcr. Mobbing psikolojik \u015fiddet, ayr\u0131\u015ft\u0131rma ve ki\u015finin kimlik de\u011ferlerine sald\u0131r\u0131lar ile karakterizedir. \u0130\u015fyerinde mobbing ise bir ki\u015fiyi hedef al\u0131p sistematik y\u0131ld\u0131rma ve ki\u015fiyi d\u00fc\u015fmanca davran\u0131\u015flara maruz b\u0131rakma \u015feklinde a\u00e7\u0131klanabilir. Hastanelerde uygulanan mobbing, ki\u015filerde fiziksel ve ruhsal hasara sebep olman\u0131n yan\u0131nda sa\u011fl\u0131k hizmet kalitesini azalt\u0131c\u0131 ve sa\u011fl\u0131k hizmetini aksat\u0131c\u0131 sonu\u00e7lar da do\u011furabilmektedir. Yap\u0131lan \u00e7al\u0131\u015fmalara g\u00f6re mobbingin astlardan \u00fcstlere, \u00fcstlerden astlara ya da uzmanl\u0131k alanlar\u0131 aras\u0131nda ge\u00e7i\u015fimli bir seyir g\u00f6sterebilece\u011fi ortaya konmu\u015f (yani herkes mobbinge u\u011frayabilir) olsa da mobbing ma\u011fdurlar\u0131n\u0131n genellikle i\u015fini iyi yapmaya \u00e7al\u0131\u015fan, yarat\u0131c\u0131 ve hastanedeki ki\u015fisel \u00e7eki\u015fmelere dahil olmay\u0131 tercih etmeyen ki\u015filerden olu\u015ftu\u011fu saptanm\u0131\u015ft\u0131r. Mobbing uygulay\u0131c\u0131lar\u0131n\u0131n ise en temel \u00f6zellikleri narsisistik e\u011filimler ta\u015f\u0131yor olmalar\u0131d\u0131r. Bu ki\u015filer, mobbinge u\u011frat\u0131lanlar\u0131n aksine do\u011fal olarak parlayamayacaklar\u0131ndan hiyerar\u015fide hep bir ad\u0131m daha ilerlemek ve kendi yetersizliklerini \u00f6rtmek i\u00e7in y\u0131ld\u0131r\u0131c\u0131 davran\u0131\u015flar sergilemekten \u00e7ekinmemekte, pozisyonlar\u0131n\u0131n getirdi\u011fi avantajlar\u0131 herkesin iyili\u011fi i\u00e7in kullanmak yerine sistemi yasal bo\u015fluklar\u0131n yol verdi\u011fi \u00f6l\u00e7\u00fcde manip\u00fcle etmeye \u00e7al\u0131\u015fmaktad\u0131rlar. T\u00fcrk Tabipler Birli\u011fi\u2019nin Ocak 2011\u2019de yay\u0131mlad\u0131\u011f\u0131 Hekimlerin \u00c7al\u0131\u015fma S\u00fcresi, N\u00f6bet, Fazla \u00c7al\u0131\u015fma \u00dccreti, Dinlenme ve \u0130zin Haklar\u0131 Kitap\u00e7\u0131\u011f\u0131\u2019na g\u00f6re hekimlerin n\u00f6bet s\u00fcrelerinin \u00fcst s\u0131n\u0131r\u0131ndaki belirsizlik, i\u015f yo\u011funlu\u011fundan depresyona girip intihara s\u00fcr\u00fcklenen, tekrarlayan g\u00fcna\u015f\u0131r\u0131 n\u00f6betlerden dolay\u0131 hayat\u0131n\u0131 kaybeden sa\u011fl\u0131k \u00e7al\u0131\u015fanlar\u0131n\u0131n \u00f6l\u00fcm\u00fcnde yasalar\u0131n rol\u00fcn\u00fc sorgulatmaktad\u0131r. Yine ayn\u0131 kitap\u00e7\u0131\u011fa g\u00f6re asistan hekimlerin kendi servisleri d\u0131\u015f\u0131nda g\u00f6revlendirilmesinin yasak olu\u015fu ve serviste mutlaka bir uzman hekimin bulunmas\u0131n\u0131n gereklili\u011fi a\u00e7\u0131k bir \u015fekilde vurgulanmas\u0131na ra\u011fmen \u00fclkenin d\u00f6rt bir yan\u0131ndaki doktorlar\u0131n verdikleri r\u00f6portajlarda uygulamadaki yanl\u0131\u015flardan yak\u0131nd\u0131klar\u0131 ve kendi deyimleriyle \u2018\u2018hastanenin y\u00fck\u00fcn\u00fc omuzlar\u0131nda ta\u015f\u0131d\u0131klar\u0131\u2019\u2019 g\u00f6r\u00fclmektedir. Yap\u0131lan ba\u015fka \u00e7al\u0131\u015fmalara g\u00f6re hiyerar\u015fik yap\u0131lanman\u0131n manip\u00fclasyonu, ki\u015filerde vicdani sorumluluk duygusunun ve empatinin zamanla k\u00f6relmesine sebep olmakta, \u00e7al\u0131\u015fma alan\u0131nda ise hiyerar\u015fik normlar\u0131n i\u00e7selle\u015ftirilmesinden kaynaklanan bir k\u0131s\u0131r zorbal\u0131k d\u00f6ng\u00fcs\u00fcne yol a\u00e7maktad\u0131r. Bu durum sa\u011fl\u0131k hizmet kalitesinin azalmas\u0131na ve dolay\u0131s\u0131yla hasta hekim ili\u015fkisinin de gerilmesine neden olmakta, h\u00e2lihaz\u0131rda var olan problemleri t\u0131rmand\u0131rmakta ve \u00fclkedeki hekime \u015fiddeti \u2018\u2018gerek\u00e7elendiren\u2019\u2019 anlay\u0131\u015f\u0131 da beslemektedir. T\u00fcrkiye\u2019deki hekimler, etnik k\u00f6kenleri, kimlikleri ve hayat tarzlar\u0131 vb. mesleki yeterliliklerinden ba\u011f\u0131ms\u0131z fakt\u00f6rlerden dolay\u0131 da ayr\u0131mc\u0131l\u0131\u011fa ve \u00e7al\u0131\u015fma hayat\u0131nda mobbinge u\u011framaktad\u0131r. Ge\u00e7ti\u011fimiz aylarda hakk\u0131nda meslekten men karar\u0131 verilen Dr. Larin Kayata\u015f Taksim E\u011fitim ve Ara\u015ft\u0131rma Hastanesi\u2019nde hal ve hareketlerinin genel ahlak(!)a uygun g\u00f6r\u00fclmedi\u011fine ili\u015fkin yaz\u0131lan dilek\u00e7eye istinaden \u00fc\u00e7 ay g\u00f6revden uzakla\u015ft\u0131r\u0131lm\u0131\u015f, sonras\u0131nda \u00f6zel hayat\u0131 ve tweetleri gerek\u00e7e g\u00f6sterilerek yarg\u0131lanm\u0131\u015ft\u0131r. Verilen uzakla\u015ft\u0131rma h\u00fckm\u00fcne a\u00e7\u0131lan kar\u015f\u0131 davada karar\u0131n iptalinin onanmas\u0131, insan onurunun korunmas\u0131 ve hekim haklar\u0131n\u0131n g\u00fcvencesi i\u00e7in umut vaat etmektedir. Sa\u011fl\u0131k sisteminde ayr\u0131\u015ft\u0131r\u0131lan bir di\u011fer grup da yabanc\u0131 uyruklu hekimlerdir. Yabanc\u0131 uyruklu hekimler mesleki atamalar\u0131n\u0131n yap\u0131lmamas\u0131 ve \u00f6zel hastanelerde ucuz i\u015f g\u00fcc\u00fc olarak \u00e7al\u0131\u015ft\u0131r\u0131lmalar\u0131n\u0131n yan\u0131 s\u0131ra toplumsal bask\u0131 ve etiketlemelere de maruz kalmaktad\u0131rlar.<\/p>\n\n\n\n<p>Sonu\u00e7 olarak sa\u011fl\u0131kta mobbingi besleyen pek \u00e7ok farkl\u0131 unsur bulunmakta, sistematik mobbing ki\u015fileri ve kurum dinamiklerini y\u0131pratmaktad\u0131r.
