Travmalar unutulabilir lakin hissettirdikleri duygu durum ki\u015finin zihninde \u00e7ak\u0131l\u0131 kal\u0131r. Bu da nedeni belirsiz, bozulmu\u015f davran\u0131\u015f bi\u00e7imine, kimi zamanda bedensel olarak somut rahats\u0131zl\u0131klara yol a\u00e7ar. Psikiyatride bu gibi durumlarda rahats\u0131zl\u0131k \u00fcreten bu duygu ve d\u00fc\u015f\u00fcncelerin ortadan kald\u0131r\u0131lmas\u0131 hedeflenir. Beynin filtrelenmesinde kullan\u0131lan bir\u00e7ok soyut ve somut y\u00f6ntem vard\u0131r. Somut y\u00f6ntemler aras\u0131nda EKT ve TMS vard\u0131r. EKT duygudurum bozukluklar\u0131 basta olmak olmak \u00fczere psikiyatrik bozukluklarda kullan\u0131lan etkinli\u011fi ispatlanm\u0131\u015f g\u00fcvenli ve daha eski bir tedavi modalitesidir. EKT\u2018nin yan etkilerinden birisi k\u0131sa s\u00fcreli ge\u00e7ici haf\u0131za kay\u0131plar\u0131d\u0131r. Bu ve benzeri yan etkileri edebiyatta veya pop\u00fcler alg\u0131da hatal\u0131 olarak s\u0131k\u00e7a i\u015flenmi\u015f ve EKT\u2019ye alternatif olarak daha g\u00fcvenli oldu\u011fu ve haf\u0131za i\u015flevleri \u00fczerine etkisi \u00f6n plana \u00e7ekilerek bir di\u011fer elektrofizyolojik m\u00fcdahale enstr\u00fcman\u0131 TMS son d\u00f6nemlerde daha fazla konu\u015fulmaya ve ara\u015ft\u0131r\u0131lmaya ba\u015flanm\u0131\u015ft\u0131r. Bu yaz\u0131mda sizlere son d\u00f6nemin pop\u00fcler konusu olan Trans Manyetik Stim\u00fclasyon tekni\u011finden ve ger\u00e7ekten faydal\u0131 olup olmad\u0131\u011f\u0131ndan bahsedece\u011fim.<\/p>\n\n\n\n<p>1985 &#8216;de ke\u015ffedilen; 2002 de Depresyon , 2007 de de Migren tedavisi i\u00e7in FDA ( Amerikan G\u0131da ve \u0130la\u00e7 Dairesi) onay\u0131 alan TMS, g\u00fcn\u00fcm\u00fczde \u00f6zellikle Diren\u00e7li Depresyon, Panik Atak, Anksiyete, Obsesif Kompulsif Bozukluk(tak\u0131nt\u0131) tedavisinde tercih ediliyor. Peki s\u00fcre\u00e7 nas\u0131l i\u015fler ?<\/p>\n\n\n\n<p>Tedavi s\u00fcresi ki\u015fiden ki\u015fiye de\u011fi\u015fmekle birlikte genelde seanslar\u0131n\u0131n her biri yakla\u015f\u0131k 20 dakika ve haftada 6 g\u00fcn uygulan\u0131r. Uyan\u0131k haldeki hastaya manyetik bir coil aparat arac\u0131l\u0131\u011f\u0131yla beynin prefrontal b\u00f6lgesine manyetik at\u0131mlar g\u00f6nderilir. \u00d6ncesinde herhangi bir premedikasyon i\u015flemi uygulanmaz. Hasta i\u015flem s\u0131ras\u0131nda herhangi bir a\u011fr\u0131 veya ac\u0131 duymaz. Beynin temel becerilerini etkileyen bir yan etkisi yoktur. TMS\u2019nin yan etkileri genellikle hafif ve k\u0131sa s\u00fcrelidir. En s\u0131k g\u00f6r\u00fclen yan etkisi ge\u00e7ici hafif ba\u015f a\u011fr\u0131s\u0131 ve uygulama b\u00f6lgesinde kar\u0131ncalanma hissidir. Bu yan etkiler genellikle tedavinin ilk haftas\u0131ndan sonra kaybolma e\u011filimindedir. Ki\u015fi nas\u0131l geldiyse ayn\u0131 \u015fekilde tedavi sonras\u0131nda hayat\u0131na devam edebilir. TMS tedavisinde haf\u0131za silinmez ,beceriler gerilemez, k\u00f6t\u00fc an\u0131lar ve travmalar s\u0131f\u0131rlanmaz. Yaln\u0131zca bunlar\u0131n bize hissettirdi\u011fi tak\u0131nt\u0131l\u0131, depresif duygulan\u0131m geriler. Tipik olarak, tedaviye ba\u015fland\u0131ktan ilk on g\u00fcn i\u00e7erisinde iyile\u015fme etkileri g\u00f6r\u00fclmeye ba\u015flar. Hastalar\u0131n \u00e7o\u011fu tedavinin ikinci haftas\u0131nda tedaviden beklenen fayday\u0131 fark etme e\u011filimindedir. Hastalar\u0131n baz\u0131lar\u0131nda yakla\u015f\u0131k 6-12 ay sonra idame (devam) tedavisine ihtiya\u00e7 olabilir. Yap\u0131lan bir ara\u015ft\u0131rmaya g\u00f6re TMS tedavisi ; Depresyonda %63 ,Obsesif Kompulsif Bozuklukta %42 ,Anksiyetede % 33,Travma Sonras\u0131 Stres Bozuklu\u011funda % 48.6 oran\u0131nda yan\u0131t alm\u0131\u015f ve bir seneyi a\u015fan s\u00fcre\u00e7lerde n\u00fckslerin ya\u015fanmaya ba\u015fland\u0131\u011f\u0131 da tespit edilmi\u015ftir.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>TMS tekni\u011fi Klasik tedavilere yeterli yan\u0131t\u0131 vermeyen \u00a0n\u00f6ropsikiyatrik bozukluklar i\u00e7in bir alternatif olarak neredeyse deneysel a\u015fama anlam\u0131nda kullan\u0131lmakta olup etkinli\u011fi psikiyatrik hastalara uygulanan psikoterapiler ve farmakoterapiler kadar y\u00fcksek de\u011fildir. \u00c7o\u011funlukla \u00f6zel merkezlerde uygulanabilen bu tedavinin g\u00fcn\u00fcm\u00fczde, ger\u00e7ek\u00a0 kullan\u0131m alan\u0131n\u0131n sapt\u0131r\u0131l\u0131p sa\u011flad\u0131\u011f\u0131 faydadan \u00e7ok daha fazlas\u0131n\u0131n vadedildi\u011fini g\u00f6rmekteyiz. Elbette belirtilen hastal\u0131klar\u0131n baz\u0131 evrelerinde tercih edilebilen bu y\u00f6ntemin var oldu\u011fu ka\u00e7\u0131n\u0131lmazd\u0131r. Fakat ne yaz\u0131k ki\u00a0 kendi i\u00e7inde olduk\u00e7a kompleks, \u00e7\u00f6z\u00fcmlenmesi zor ve me\u015fakkatli bir bran\u015f olan Psikiyatri&#8217;ye yan daldan dahil olmaya \u00e7al\u0131\u015fan ,insanlar\u0131n \u00fcmitlerini s\u00f6m\u00fcren bir kesim taraf\u0131ndan suistimal edildi\u011fi ger\u00e7e\u011fi de yads\u0131namaz. \u0130\u015fin tuhaf\u0131 bir anda bu kadar\u00a0 pop\u00fclerle\u015fen bu metod psikiyatrik rahats\u0131zl\u0131\u011f\u0131 olmaks\u0131z\u0131n ac\u0131 \u00e7eken, \u00fcz\u00fcnt\u00fc duyan, unutmak isteyen herkesin de g\u00fcndeminde&#8230; Bir anda b\u00fct\u00fcn an\u0131lar\u0131n\u0131n silinebilece\u011fi, i\u00e7inde bulundu\u011fu buhrandan tek bir y\u00f6ntemle kurtulabilece\u011fini sanan bir kesim mevcut. Halbuki unutman\u0131n asla ama asla m\u00fcmk\u00fcn olmad\u0131\u011f\u0131, yaln\u0131zca al\u0131\u015fman\u0131n m\u00fcmk\u00fcn oldu\u011fu ger\u00e7e\u011fiyle y\u00fczle\u015filmeli ve zihnin daima kay\u0131t tuttu\u011fu bilinmelidir. Beyin \u00f6yle bir olu\u015fumdur ki ondan hi\u00e7bir \u015feyi silemez ve o kendi istemedi\u011fi s\u00fcrece ona hi\u00e7bir \u015feyi unutturamazs\u0131n\u0131z. Ancak an\u0131lar\u0131 ve onda b\u0131rakt\u0131\u011f\u0131 izleri d\u00f6n\u00fc\u015ft\u00fcrebilir, kabullenebilir ve sahiplenebilir hale getirebilirsiniz. \u0130nsanlar\u0131n travmalar sonucunda veya travmalar olmaks\u0131z\u0131n sahip olduklar\u0131 psikiyatrik bozukluklar uygun, etkinli\u011fi ve g\u00fcvenilirli\u011fi\u00a0 bilimsel ara\u015ft\u0131rmalarla ortaya konulmu\u015f tedavi bi\u00e7imleri ile tedavi edilmeye \u00e7al\u0131\u015f\u0131l\u0131r.
