G\u00f6z\u00fcn tabakalar\u0131 s\u0131ras\u0131 ile ; <\/p>\n\n\n\n<p>-D\u0131\u015f tabaka (tunica fibrosa): sklera ve kornea,<\/p>\n\n\n\n<p>-Orta tabaka (tunica vasculosa): koroid, corpus ciliare, iris, <\/p>\n\n\n\n<p>-\u0130\u00e7 tabaka (tunica nervosa): retina\u2019d\u0131r.<br> Retina g\u00f6z\u00fcn arka tabakas\u0131n\u0131 kaplayan kompleks \u00e7ok katmanl\u0131 bir sinir yap\u0131s\u0131d\u0131r. Bruch membran\u0131 (BM), koroid ve alt\u0131ndaki destekleyici kat olarak sklera ile n\u00f6ral retina ve retina pigment epitelinden (RPE) olu\u015fur. RPE fotoresept\u00f6r h\u00fccreleri ile koryokapillaris tabakas\u0131 aras\u0131nda yer alan ve bir\u00e7ok a\u00e7\u0131dan \u00f6zelle\u015fmi\u015f epitel h\u00fccreleridir. N\u00f6ral retina, fotoresept\u00f6r h\u00fccreler dahil olmak \u00fczere bir\u00e7ok farkl\u0131 h\u00fccre tiplerini i\u00e7erir ve birden \u00e7ok fonksiyona sahiptir. I\u015f\u0131k retina taraf\u0131ndan yans\u0131t\u0131ld\u0131\u011f\u0131nda, farkl\u0131 retinal h\u00fccre tipleri n\u00f6rokimyasal ve optik sinir ile beyine g\u00f6nderilen elektrik sinyalleri \u0131\u015f\u0131\u011f\u0131 d\u00f6n\u00fc\u015ft\u00fcrmek \u00fczere birlikte \u00e7al\u0131\u015f\u0131r. Birinci ve bu sistemde en \u00f6nemli bile\u015fenlerinden biri olan fototransd\u00fcksiyon kaskad\u0131, fotoresept\u00f6r h\u00fccrelerinin \u0131\u015f\u0131k duyarl\u0131l\u0131\u011f\u0131n\u0131 s\u00fcrd\u00fcren bir biyokimyasal sistemidir. Fototransd\u00fcksiyon kaskad\u0131nda rol oynayan proteinler hem RPE de hem de fotoresept\u00f6rlerde bulunur. Bu proteinleri \u015fifreleyen genler RPE ve fotoresept\u00f6r h\u00fccrelerinde y\u00fcksek seviyelerde ifade edilir. Retina boyunca gelen ve giden molek\u00fcller ve elektrolitlerin hassas bir homeostaz\u0131 vard\u0131r. K\u0131smen RPE ve fotoresept\u00f6rlerde mevcut ifade edilen genler ve proteinler taraf\u0131ndan d\u00fczenlenirler. Etkilenen yeni do\u011fanlar do\u011fduklar\u0131nda \u00e7o\u011funlukla k\u00f6rlerdir. Bebeklikten itibaren LCA olan insanlarda a\u011f\u0131r g\u00f6rme bozuklu\u011fu vard\u0131r. Bu bozukluk ilerleyici de\u011fildir ama \u00e7ok yava\u015f bir \u015fekilde zamanla k\u00f6t\u00fcle\u015febilir.<\/p>\n\n\n\n<p>Do\u011fumda g\u00f6rsel tepkinin azalmas\u0131 hastal\u0131\u011f\u0131n ilk i\u015faretidir. ERG bu hastal\u0131kta da kullan\u0131l\u0131yor ve Leber Conjenital Amaurousis (LCA) olan bebeklerde retinan\u0131n elektriksel aktivitesinin olmad\u0131\u011f\u0131 ya da azald\u0131\u011f\u0131 g\u00f6zleniyor. Molek\u00fcler genetik testler ise LCA  ile ili\u015fkili genler i\u00e7in m\u00fcmk\u00fcnd\u00fcr. Klinik bulgular, hangi genlerin ve hangi s\u0131rayla test edileceklerinin belirlenmesinde yard\u0131mc\u0131d\u0131r.<\/p>\n\n\n\n<p><strong>Antenatal Te\u015fhis<\/strong><\/p>\n\n\n\n<p>Do\u011fum \u00f6ncesinde risk grubunda bulunan yani hastal\u0131\u011fa sebep olan mutasyonlar\u0131n belirlendi\u011fi \u00e7iftler i\u00e7in \u00f6zelle\u015fmi\u015f laboratuarlar taraf\u0131ndan prenatal te\u015fhis \u00f6nerilebilir.<\/p>\n\n\n\n<p><br><\/p>\n\n\n\n<p><strong>Tedavi<\/strong><\/p>\n\n\n\n<p>Leber konjenital amorozisi i\u00e7in \u015fu anda bir k\u00fcr bulunmuyor yani kesin tedavisi yok. Tedaviler semptomatik ve destekleyici olarak yap\u0131lmakta. Hasta olan \u00e7ocuklar\u0131n devaml\u0131 olarak g\u00f6zlerini ovmamalar\u0131n\u0131n ve ellerini g\u00f6zlerine bast\u0131rmamalar\u0131n\u0131n da sa\u011flanmas\u0131 gerekiyor. Periyodik olarak g\u00f6z tembelli\u011fi, glokom veya katarakt i\u00e7in oftalmik kontrollerin yap\u0131lmas\u0131 da \u00f6nem arz ediyor. Bunlar 4 ana ba\u015fl\u0131k alt\u0131nda incelenebilir.<br> 1) Fotoresept\u00f6r apoptozisini engellemek i\u00e7in kullan\u0131lan anti-apoptotik gen tedavisi<br> 2) Bir\u00e7ok farkl\u0131 tipteki RP modeli i\u00e7in kullan\u0131lan n\u00f6rotropin\/growth fakt\u00f6r gen tedavisi<br> 3) Dominant mutant proteinlerin toksik etkilerini bloke etmek i\u00e7in kullan\u0131lan ribozom gen tedavisi<br> 4) Eksik genin yerine konulmas\u0131 i\u00e7in kullan\u0131lan gen yenileme tedavisidir.<br> Ancak temel strateji, OR ve X&#8217;e ba\u011fl\u0131 formlarda eksik genin yerine konulmas\u0131, OD formlarda ise mutant proteinin ekspresyonunu bloke edilmesi \u015feklindedir.<br> Son 30 y\u0131lda geli\u015ftirilen Rekombinant DNA teknolojisi ile iki farkl\u0131 kaynaktan al\u0131nan DNA\u2019lar\u0131n birle\u015ftirilmesi m\u00fcmk\u00fcn olmu\u015ftur. Burada aktar\u0131lmas\u0131 istenen belli bir DNA b\u00f6lgesi (\u00f6rne\u011fin insan Rb geni) onu h\u00fccreye ta\u015f\u0131yacak uygun bir ta\u015f\u0131y\u0131c\u0131 vekt\u00f6re (bir DNA vir\u00fcs\u00fc) aktar\u0131l\u0131r. Bu yolla gen tedavi protokolleri olu\u015fturulmu\u015f ve deneysel bazda uygulama alan\u0131 bulmu\u015ftur. Burada aktar\u0131lmak istenen geni istenilen h\u00fccrelere g\u00f6t\u00fcrmesi gereken en uygun viral vekt\u00f6r\u00fcn yap\u0131lmas\u0131 ba\u015far\u0131 i\u00e7in en \u00f6nemli parametredir. Halen s\u00fcren \u00e7al\u0131\u015fmalarda bu sonu\u00e7lar\u0131 daha iyiye ta\u015f\u0131yacak vekt\u00f6r dizaynlar\u0131 \u00fczerinde durulmaktad\u0131r. Bu yolla elde edilecek bilgiler g\u00f6z hastal\u0131klar\u0131nda uygulama alan\u0131na girebilecek ileri tan\u0131 ve tedavi protokollerinin bulunmas\u0131n\u0131 sa\u011flayacakt\u0131r.
