Bunun sebebi, laktoz intolerans\u0131 olan bireylerin s\u00fct ve s\u00fct \u00fcr\u00fcnlerinin i\u00e7erisinde bulunan laktoz ad\u0131ndaki \u015fekeri tamamen sindirememeleridir. Bu y\u00fczden bu hastalarda s\u00fct ve s\u00fct \u00fcr\u00fcnleri t\u00fcketimi sonras\u0131nda diyare (ishal), gaz ve \u015fi\u015fkinlik g\u00f6r\u00fcl\u00fcr. Bunun sebebini fizyoloji derslerimizde g\u00f6rd\u00fc\u011f\u00fcm\u00fcz, hipotonik-hipertonik ortam mant\u0131\u011f\u0131yla anlayabiliriz. Ba\u011f\u0131rsak l\u00fcmeninde sindirilememi\u015f laktoz bulundu\u011fundan, artan ozmotik bas\u0131nca kar\u015f\u0131l\u0131k olarak l\u00fcmene s\u0131v\u0131 salg\u0131s\u0131n\u0131n artt\u0131\u011f\u0131n\u0131 d\u00fc\u015f\u00fcnebiliriz.<\/p>\n\n\n\n<p>Laktoz intolerans\u0131, ince ba\u011f\u0131rsaklar\u0131n yeterince laktaz enzimi salg\u0131lamamas\u0131 sonucu olu\u015fur. Normalde laktaz sayesinde laktoz; glukoz ve galaktoz ad\u0131nda 2 adet monosakkarite d\u00f6n\u00fc\u015f\u00fcr ve b\u00f6ylece ba\u011f\u0131rsak duvar\u0131ndan emilerek kan dola\u015f\u0131m\u0131na kar\u0131\u015f\u0131r. Eri\u015fkin ya\u015famda ise normalde minimum d\u00fczeyde de olsa g\u00fcnl\u00fck s\u00fct \u00fcr\u00fcn\u00fc t\u00fcketimimizi sindirmeye yetecek miktarda laktaz \u00fcretimi olmas\u0131n\u0131 bekleriz. Fakat primer hipolaktazi hastalar\u0131nda bir allel gen eksikli\u011finden dolay\u0131 <em>(lactase persistence allele)<\/em> (2,3) eri\u015fkin ya\u015famda laktaz \u00fcretimi \u00e7ok sert bir \u015fekilde d\u00fc\u015fer ve hipolaktazi durumu g\u00f6r\u00fcl\u00fcr. Genetik olarak kodlanan bu durum, okudu\u011fum birka\u00e7 kaynakta farkl\u0131 belirtilmekle birlikte, en \u00e7ok Afrika, Asya ve Hispanik k\u00f6kenli insanlarda g\u00f6r\u00fcl\u00fcyor fakat primer hipolaktazinin ayn\u0131 zamanda Akdeniz ve Do\u011fu Avrupa k\u00f6kenli insanlarda da yayg\u0131n olarak g\u00f6r\u00fcld\u00fc\u011f\u00fcn\u00fc s\u00f6yleyebiliriz. Sekonder hipolaktazi hem infant hem de eri\u015fkinlerde g\u00f6r\u00fclebilir ve primer hipolaktazinin aksine genellikle geri d\u00f6nd\u00fcr\u00fclebilir\/iyile\u015ftirilebilirdir. Sekonder laktoz intolerans\u0131; akut gastroenterit, \u00e7\u00f6lyak hastal\u0131\u011f\u0131, Crohn hastal\u0131\u011f\u0131, \u00fclseratif kolit, kemoterapi, giardia gibi ba\u011f\u0131rsak parazitleri veya ba\u015fka \u00e7evresel fakt\u00f6rler kaynakl\u0131 olabilir (2,5,6). Sekonder hipolaktaziye sebep olan hastal\u0131klara kar\u015f\u0131 tedavi uyguland\u0131\u011f\u0131 zaman laktaz \u00fcretimi eski h\u00e2line d\u00f6nd\u00fcr\u00fclebilir.<\/p>\n\n\n\n<h4>3.Primer Konjenital Alaktazi\/Konjenital Laktoz \u0130ntolerans\u0131:<\/h4>\n\n\n\n<p>Olduk\u00e7a nadir, otozomal resesif enzim defektidir ve bebeklerde do\u011fumla birlikte laktaz ekspresyonunu engeller. Alaktazi dedi\u011fimiz laktaz enziminin hi\u00e7 \u00fcretilmemesi s\u00f6z konusudur. Bu y\u00fczden bu bebekler s\u00fct\u00fc hi\u00e7 sindiremezler (2).
