Genellikle kahvenin g\u00fcn\u00fcm\u00fcze ba\u015flamam\u0131za yard\u0131m etmesi, i\u00e7indeki etken madde kafeinden kaynaklanmaktad\u0131r. Kafein, D\u00fcnya&#8217;da en fazla t\u00fcketilen psiko-aktif maddedir (2). Ancak ayn\u0131 kategoride bulundu\u011fu bir\u00e7ok maddeden farkl\u0131 olarak da kullan\u0131m\u0131 neredeyse denetlenmemektedir. Kafein, bir\u00e7ok alanda yayg\u0131n olarak kullan\u0131lan bir maddedir. Bu duruma \u00f6rnek olarak ba\u015f a\u011fr\u0131lar\u0131na kar\u015f\u0131 kullan\u0131lan, Aspirin benzeri ila\u00e7lar\u0131n i\u00e7inde bulunmas\u0131, ast\u0131m tedavisinde kullan\u0131lmas\u0131 ve i\u015ftah azalt\u0131c\u0131 maddelerin i\u00e7ine eklenmesi verilebilir (2). Peki, kafein bunu nas\u0131l ger\u00e7ekle\u015ftirmektedir? Bu konuda yap\u0131lan ara\u015ft\u0131rmalar\u0131n verdi\u011fi sonu\u00e7lar\u0131 yorumlamak \u00e7ok kar\u0131\u015f\u0131k bir \u00e7al\u0131\u015fmad\u0131r ve yap\u0131lan bir\u00e7ok ara\u015ft\u0131rma da kesin sonu\u00e7 vermemektedir (2, 3, 4). Bunun en b\u00fcy\u00fck nedeni de kafeinin insanlar taraf\u0131ndan genellikle \u00e7ay, kahve, kola gibi i\u00e7ecekler i\u00e7inde t\u00fcketilmesi ve dolay\u0131s\u0131yla etkisinin bu i\u00e7ecekler i\u00e7inde bulunan di\u011fer etken maddeler taraf\u0131ndan da de\u011fi\u015ftirilebilece\u011fidir (2). Ancak bu \u015fekilde bir etkinin yarat\u0131labilmesi i\u00e7in kafeinin plazma konsantrasyonun en az 250 mikromolar olmas\u0131 gerekmektedir. Kahve i\u00e7tikten sonra bir insan\u0131n v\u00fccudundaki bu de\u011ferin 100 mikromolardan az oldu\u011fu bilinmektedir. Dolay\u0131s\u0131yla, kahveden gelen kafeinin hayat\u0131m\u0131zda \u00f6nemli bir yer etmesine yol a\u00e7an etkisi bu de\u011fildir (2). Kafeinin bir di\u011fer etkisi de, Nehlig ve ark.\u2019\u0131n \u00f6zetlemi\u015f oldu\u011fu gibi, fosfodiesterazlar\u0131 engellemesidir. Bu i\u015flevin \u00e7ok \u00f6nem kazand\u0131\u011f\u0131 bir v\u00fccut k\u0131sm\u0131 da merkezi sinir sistemidir. Burada kafeinin siklik n\u00fckleotitlere benzer bir yap\u0131ya sahip oldu\u011fu i\u00e7in enzimlerin inhibit\u00f6rl\u00fc\u011f\u00fcn\u00fc yapt\u0131\u011f\u0131 g\u00f6zlemlenmi\u015ftir. Ancak yine bir \u00f6nceki etki gibi bu sefer de kafeinin etkili olabilmesi i\u00e7in toksik bir d\u00fczeyde bulunmas\u0131 gerekti\u011fi de kan\u0131tlanm\u0131\u015ft\u0131r. Dolay\u0131s\u0131yla denebilir ki, g\u00fcnl\u00fck 25 mg\/kg\u2019lik bir kafein t\u00fcketimi, h\u00fccre i\u00e7indeki cAMP konsantrasyonunu art\u0131r\u0131p fosfodiesteraz aktivitesini de\u011fi\u015ftirmek i\u00e7in yeterli de\u011fildir (7). S\u0131rada, bilim insanlar\u0131n\u0131n kafeinin kullan\u0131\u015fl\u0131 etkilerinin alt\u0131nda yatabilecek mekanizma olarak g\u00f6rd\u00fckleri adenozin resept\u00f6rlerini engelleyebilmesi var. Bu hipotezler, asl\u0131nda Sattin ve Rall\u2019\u0131n laboratuvar \u00e7al\u0131\u015fmalar\u0131na dayan\u0131r. Bunu anlayabilmek i\u00e7in \u00f6ncelikle adenozinin in vivo etkilerini inceleyelim. Genel olarak bak\u0131ld\u0131\u011f\u0131nda adenozin ve t\u00fcrevleri, kafeine ters etkiler yaratmaktad\u0131r. Bu etkilere \u00f6rnek olarak, sinir h\u00fccrelerinin elektriksel aktivitelerinin ve n\u00f6rotransmiter salg\u0131lanmas\u0131n\u0131n engellenmesi verilebilir. Adenozinin hareketin \u00f6n\u00fcne ge\u00e7en etkileri, kafein t\u00fcketimiyle engellenebilir. Bunun \u00f6ncelikli nedeni de daha \u00f6nce belirtildi\u011fi gibi, kafein maddesinin adenozin resept\u00f6rlerine rahat\u00e7a ba\u011flanarak normalde ger\u00e7ekle\u015fecek durumun tam tersini yaratabilmeleridir. Kafeinin bu etkisini kullanan \u00f6nemli bir grup da dayan\u0131kl\u0131l\u0131klar\u0131n\u0131 art\u0131rmak isteyen sporculard\u0131r (8). Belki de \u00e7o\u011fumuz kafein i\u00e7eren i\u00e7ecekleri uzun s\u00fcre ders \u00e7al\u0131\u015ft\u0131\u011f\u0131m\u0131z zamanlarda bizlere biraz keyif ve enerji vermesi i\u00e7in t\u00fcketiyoruz, belki de art\u0131k bizler i\u00e7in tamamen bir al\u0131\u015fkanl\u0131k haline geldi\u011finden bunlar\u0131 t\u00fcketirken d\u00fc\u015f\u00fcnm\u00fcyoruz bile. Fakat kafein t\u00fcketimimiz sadece bir al\u0131\u015fkanl\u0131k m\u0131d\u0131r yoksa bunun alt\u0131nda yatan ba\u015fka mekanizmalar da var m\u0131? Baz\u0131 insanlar\u0131n ancak birka\u00e7 tane kahve i\u00e7tikten sonra etkilerini hissetmesi sadece fazla kahve i\u00e7meye al\u0131\u015fmakla m\u0131 yoksa genetik parametrelere de ba\u011fl\u0131 m\u0131d\u0131r? Bu sorular, y\u0131llard\u0131r bir\u00e7ok bilim insan\u0131n\u0131n cevab\u0131n\u0131 arad\u0131\u011f\u0131 ve ara\u015ft\u0131rd\u0131\u011f\u0131 sorulard\u0131r.&nbsp;<\/p>\n\n\n\n<p style=\"text-align:center\">\u00d6ncelikle, kafeinin ba\u011f\u0131ml\u0131l\u0131k yapan bir madde olmak i\u00e7in gereken b\u00fct\u00fcn kriterleri sa\u011flad\u0131\u011f\u0131n\u0131 belirtmek gerekiyor (9). Ayn\u0131 zamanda kafeinin v\u00fccuttaki etkileri, ilk defa kullanan bir insan ve kafein kullanmaya al\u0131\u015fk\u0131n bir insan aras\u0131nda da farkl\u0131l\u0131k g\u00f6stermektedir, dolay\u0131s\u0131yla kafein t\u00fcketiminde de bir tolerans e\u015fi\u011fi bulunmaktad\u0131r (10). Dayan\u0131kl\u0131l\u0131k \u00fczerine \u00e7al\u0131\u015fan sporcularda yap\u0131lan \u00e7e\u015fitli ara\u015ft\u0131rmalar, kafeinin etkileri hakk\u0131nda birbirlerinden farkl\u0131 ve bazen de beklenmedik sonu\u00e7lar vermektedir (8). Kafeinin performans\u0131 art\u0131r\u0131c\u0131 etkilerine g\u00f6stermesinde ise metabolizmas\u0131, ne zaman t\u00fcketildi\u011fi, hangi dozda t\u00fcketildi\u011fi, hangi \u015fekilde t\u00fcketildi\u011fi gibi bir\u00e7ok fakt\u00f6r belirleyici olmaktad\u0131r. Kafein t\u00fcketimine ve metabolizmas\u0131na verilen biyolojik tepkilerde \u00f6nemli rol oynayan genler, CYP1A2 ve ADORA2A genleridir (11).&nbsp;<\/p>\n\n\n\n<p style=\"text-align:center\">\u00d6ncelikle CYP1A2\u2019den bahsedelim. Bu protein, v\u00fccudumuzdaki ila\u00e7 metabolizmas\u0131n\u0131n %75\u2019inden sorumlu olan sitokrom P450 enzim ailesinin bir \u00fcyesidir (12). CYP1A2 enzimi, kafein metabolizmas\u0131n\u0131n %90\u2019\u0131ndan daha fazlas\u0131ndan sorumludur (8). Dayan\u0131kl\u0131l\u0131k egzersizlerini d\u00fc\u015f\u00fcnecek olursak, bu sporcular\u0131n daha yava\u015f ilerleyen bir kafein metabolizmas\u0131na sahip olmalar\u0131 ve bu sayede uzun s\u00fcren fiziksel aktiviteleri (maraton, triatlon gibi) boyunca bu etkilerden yararlanarak din\u00e7 kalmalar\u0131 onlar i\u00e7in daha faydal\u0131 olacakt\u0131r. CYP1A2 geni d\u00fc\u015f\u00fcn\u00fcld\u00fc\u011f\u00fcnde, genetik olarak iki tane alel tespit edilmi\u015ftir. Bunlar \u2018h\u0131zl\u0131 sindirici\u2019 A ve \u2018yava\u015f sindirici\u2019 C alelidir. Sadece A\/A genotipini bulunduranlar h\u0131zl\u0131 metabolize edenler olarak de\u011ferlendirilmektedir. Ancak bu s\u0131n\u0131fland\u0131rmaya ra\u011fmen yap\u0131lan ara\u015ft\u0131rmalar birbirlerine z\u0131t ve bu gen ile kafein metabolizmas\u0131 aras\u0131ndaki ili\u015fkiye dair kesin bir bilgiye ula\u015fmam\u0131z\u0131 engelleyecek \u015fekilde sonu\u00e7 vermektedir.&nbsp;<\/p>\n\n\n\n<p style=\"text-align:center\">Kafein metabolizmas\u0131nda etkili oldu\u011funu belirtti\u011fim ikinci gen ADORA2A ise A2A adl\u0131 ve yo\u011fun olarak beyinde bulunan adenozin resept\u00f6r\u00fcn\u00fc kodlamaktad\u0131r. Ayn\u0131 zamanda bu resept\u00f6r, dopamin ve glutamin n\u00f6rotransmiterlerinin h\u00fccre d\u0131\u015f\u0131na b\u0131rak\u0131lmas\u0131n\u0131 da engellemektedir. Yap\u0131lan bir\u00e7ok ara\u015ft\u0131rmaya g\u00f6re, bu genin sentezledi\u011fi protein, al\u0131\u015fkanl\u0131k olarak t\u00fcketilen kafein miktar\u0131n\u0131 etkilemektedir. \u00d6rne\u011fin, Cornelis ve ark. taraf\u0131ndan y\u00fcr\u00fct\u00fclen bir ara\u015ft\u0131rma, CYP1A geninin g\u00fcnl\u00fck kafein t\u00fcketiminde etkisi olmazken ADORA2A geninde T\/T genotipine sahip olan ki\u015filerin C aleline sahip olan ki\u015filere g\u00f6re daha az kafein t\u00fcketti\u011fi sonucunu ortaya koymu\u015ftur (13). Benzer ba\u015fka bir ara\u015ft\u0131rma da T\/T genotipli bireylerde kafeinden kaynaklanan anksiyetenin daha \u015fiddetli oldu\u011funu ortaya koymu\u015ftur (14). Bu konuda yap\u0131lan bir\u00e7ok ara\u015ft\u0131rma birbirlerini desteklemekte ve T\/T genotipli insanlar\u0131n biyolojik negatif geri bildirim nedeniyle daha az kafein t\u00fckettiklerini d\u00fc\u015f\u00fcnd\u00fcrmektedir (8).&nbsp;<\/p>\n\n\n\n<p style=\"text-align:center\">Peki, genlerin kafein metabolizmas\u0131na etkisinden sonra kafeine ba\u011f\u0131ml\u0131l\u0131\u011f\u0131 nas\u0131l etkiledi\u011fini inceleyelim. Mills ve ark. (2017), bu konuda ilgin\u00e7 bir tespitte bulundu. Y\u00fcr\u00fctt\u00fckleri ara\u015ft\u0131rmaya g\u00f6re, bir sa\u011fl\u0131k durumunun genetik altyap\u0131s\u0131n\u0131n bireyler taraf\u0131ndan bilinmesi baz\u0131 durumlarda bu ki\u015filerde, bahsi ge\u00e7en sa\u011fl\u0131k durumunun de\u011fi\u015ftirilemez oldu\u011fu \u015feklinde bir alg\u0131 yarat\u0131yordu (15). Bunun \u00fczerine de bireyler, sa\u011fl\u0131k durumlar\u0131n\u0131 de\u011ferlendirme bi\u00e7imlerini de\u011fi\u015ftiriyorlard\u0131. Bu g\u00f6zlemi asl\u0131nda \u015fu \u015fekilde de ifade edebiliriz: e\u011fer siz genetik olarak bir hastal\u0131\u011fa yatk\u0131n oldu\u011funuzu \u00f6\u011frenirseniz, bu hastal\u0131\u011f\u0131 daha hafif atlatabilmek veya hi\u00e7 ge\u00e7irmemek amac\u0131yla \u00e7aba g\u00f6sterebilirsiniz; bu hastal\u0131k hakk\u0131nda daha fazla bilgi edinmek ve d\u00fc\u015f\u00fcnmek isteyebilirsiniz (Aktan-Collan, aktaran Mills, 2017). Ancak buna ters etkileri ya\u015fayan ki\u015filer de g\u00f6r\u00fclm\u00fc\u015ft\u00fcr. \u00d6rne\u011fin Lineweaver ve ark. taraf\u0131ndan y\u00fcr\u00fct\u00fclen bir ara\u015ft\u0131rmada; Alzheimer hastal\u0131\u011f\u0131 i\u00e7in genetik test sonu\u00e7lar\u0131 pozitif olan kat\u0131l\u0131mc\u0131lar, sonu\u00e7lar\u0131n\u0131n pozitif oldu\u011fu s\u00f6ylenen ve s\u00f6ylenmeyen \u015feklinde iki gruba ayr\u0131lm\u0131\u015ft\u0131r. Bu s\u00fcrecin sonunda yap\u0131lan bir haf\u0131za testinde pozitif oldu\u011funu bilen ki\u015filer di\u011ferlerinden daha k\u00f6t\u00fc sonu\u00e7 alm\u0131\u015f ve ayn\u0131 zamanda kendi haf\u0131za becerilerini de di\u011ferlerinden daha sert bir \u015fekilde kritik etmi\u015flerdir (17). Buna benzer etkiler kemoterapi alan hastalar\u0131n, ila\u00e7lar\u0131n muhtemel yan etkilerini okuduktan sonra bu yan etkileri ya\u015fad\u0131klar\u0131n\u0131 daha s\u0131k bir \u015fekilde ifade etmelerinde de g\u00f6r\u00fclm\u00fc\u015ft\u00fcr (18).
