{"id":8573,"date":"2020-11-21T13:11:36","date_gmt":"2020-11-21T10:11:36","guid":{"rendered":"http:\/\/blog.ulubat.org\/?p=8573"},"modified":"2020-11-21T13:11:38","modified_gmt":"2020-11-21T10:11:38","slug":"nano-teknoloji-bakteri-direncinin-sonu-olabilir-mi","status":"publish","type":"post","link":"https:\/\/blog.ulubat.org\/index.php\/genel\/nano-teknoloji-bakteri-direncinin-sonu-olabilir-mi\/","title":{"rendered":"NANO TEKNOLOJ\u0130 BAKTER\u0130 D\u0130RENC\u0130N\u0130N SONU OLAB\u0130L\u0130R M\u0130"},"content":{"rendered":"\n<p>Bug\u00fcnlerde \u00e7ok basit hastal\u0131klar olarak d\u00fc\u015f\u00fcnd\u00fc\u011f\u00fcm\u00fcz, kolay tedavi edilebilen ishal ve pn\u00f6moni gibi bakteri etkenli hastal\u0131klar\u0131n ge\u00e7mi\u015fte \u00e7o\u011fu insan i\u00e7in bir \u00f6l\u00fcm sebebi oldu\u011funu biliyoruz.\u00a0 Bakterilerle olan bu sava\u015f\u0131m\u0131z\u0131n \u00f6nc\u00fcs\u00fcn\u00fcn Alexander Fleming ve onun yanl\u0131\u015fl\u0131kla ke\u015ffetti\u011fi penisilin oldu\u011fu genel bir d\u00fc\u015f\u00fcnce olsa da anti bakteriyel materyallerin ara\u015ft\u0131r\u0131lmas\u0131 \u00e7ok daha eski tarihlere dayan\u0131r. \u00d6rne\u011fin eski M\u0131s\u0131rl\u0131lar enfekte olmu\u015f yaralar i\u00e7in k\u00fcfl\u00fc ekmek kullan\u0131yorlard\u0131. <\/p>\n\n\n\n<p>Modern antibiyoti\u011fin ilk \u00f6rne\u011fi ise k\u00e2\u015fifi Paul Erhlich\ntaraf\u0131ndan \u201ckemoterapi\u201d olarak adland\u0131r\u0131lan, sifiliz tedavisinde kullan\u0131lm\u0131\u015f\nArsfenamin idi. \u201cAntibiyotik\u201d terimi ise ilk kez; ya\u015fam\u0131 boyunca 20\u2019den fazla\nantibiyotik ila\u00e7 ke\u015ffeden Selman Waksman taraf\u0131ndan kullan\u0131ld\u0131. <\/p>\n\n\n\n<p>Daha sonra ke\u015fiflerin ard\u0131 arkas\u0131 kesilmedi. Farkl\u0131 mekanizmalarla bakterileri etkisiz k\u0131lan onlarca antibiyotik t\u00fcr\u00fc geli\u015ftirildi. Bu s\u00fcre\u00e7te milyonlarca ki\u015finin hayat\u0131n\u0131n kurtuldu\u011funu s\u00f6ylesek san\u0131r\u0131m abartm\u0131\u015f olmay\u0131z. Her \u015fey \u00e7ok g\u00fczel gidiyordu. Etkisi olmasa bile viral hastal\u0131klar i\u00e7in dahi bu ila\u00e7lar\u0131 kullan\u0131yorduk. Sava\u015f\u0131n mutlak galibi oldu\u011fumuzu d\u00fc\u015f\u00fcn\u00fcrken bakterilerin de bo\u015f durmad\u0131\u011f\u0131n\u0131 fark ettik ve \u00e7a\u011f\u0131m\u0131z\u0131n en b\u00fcy\u00fck halk sa\u011fl\u0131\u011f\u0131 tehlikelerinden biri olan \u201cBakteri Direnci\u201d ile kar\u015f\u0131la\u015ft\u0131k. <\/p>\n\n\n\n<p>\u00c7ark tersine d\u00f6nd\u00fc ve bir\u00e7ok hasta, diren\u00e7li bakteriler\nnedeniyle kaybedilmeye ba\u015fland\u0131. Hatta D\u00fcnya Sa\u011fl\u0131k \u00d6rg\u00fct\u00fc 2013\u2019te bu g\u00fczel\nd\u00f6nemin sona erdi\u011fini ve \u201cpost antibiyotik\u201d d\u00f6nemin \u00e7oktan ba\u015flad\u0131\u011f\u0131n\u0131 duyurdu.\n<\/p>\n\n\n\n<p>Bilim camias\u0131 ise bu soruna \u00e7areler ar\u0131yor. G\u00f6sterilen dirence kar\u015f\u0131 daha g\u00fc\u00e7l\u00fc antibiyotikler \u00fcretilmesi y\u00f6ntemlerden biriydi. Ancak bunlara kar\u015f\u0131 da diren\u00e7 g\u00f6steren bakteriler oldu. Ayr\u0131ca spektrum geni\u015fledik\u00e7e ilac\u0131n yan etkileri de artmaya ba\u015flad\u0131 ve mikrobiyotan\u0131n zarar g\u00f6rd\u00fc\u011f\u00fc g\u00f6zlemlendi. Ek olarak y\u00fcksek maliyetli yat\u0131r\u0131mlar olmas\u0131na ra\u011fmen sonu\u00e7 al\u0131namama ihtimali farmas\u00f6tik \u015firketlerini cezbetmedi. Bir di\u011fer \u00e7\u00f6z\u00fcm, birlikte kullan\u0131ld\u0131\u011f\u0131nda antibiyotiklerin etkisini art\u0131ran \u201cadjuvan\u201d lar\u0131n \u00fcretimiydi. <\/p>\n\n\n\n<p>Bir y\u00f6ntem daha var ki ayr\u0131nt\u0131lar\u0131n\u0131 \u00f6\u011frendik\u00e7e insan kendini Jetgiller\u2019de gibi hissediyor &#x1f60a; Evet hayat\u0131n bir\u00e7ok alan\u0131nda etkin olarak kullan\u0131lmaya ba\u015flanan nano teknoloji ile bu defa bakterilerle olan sava\u015f\u0131m\u0131zda kar\u015f\u0131la\u015f\u0131yoruz. <\/p>\n\n\n\n<p>Nano boyuttaki materyaller, genel olarak bakterilerin diren\u00e7 mekanizmalar\u0131n\u0131 k\u0131rmay\u0131 ama\u00e7lar. Bu diren\u00e7 mekanizmalar k\u0131saca;<\/p>\n\n\n\n<ol><li>antibiyotiklerin ba\u011flanarak etki g\u00f6sterdi\u011fi\nhedef b\u00f6lgelerin de\u011fi\u015ftirilmesi<\/li><li>ila\u00e7 etkisini inhibe edecek madde sekresyonu<\/li><li>Pompa sistemleriyle ilac\u0131n etkisini g\u00f6steremeden\nh\u00fccre d\u0131\u015f\u0131na at\u0131l\u0131m\u0131<\/li><li>Biyofilm olu\u015fumu <\/li><\/ol>\n\n\n\n<p>\u015feklinde \u00f6zetlenebilir. \u00d6zellikle biyofilm \u00e7ok g\u00fc\u00e7l\u00fc bir diren\u00e7 nedenidir. Bakterilerin bu yolla antibiyotiklere kar\u015f\u0131 1000 kat daha dayan\u0131kl\u0131 hale gelebildi\u011fi biliniyor.\u00a0  <\/p>\n\n\n\n<figure class=\"wp-block-image\"><img src=\"https:\/\/www.reactgroup.org\/wp-content\/uploads\/2016\/09\/Resistance-mechanisms_Erik-Gullberg.png\" alt=\"\" \/><figcaption>Diren\u00e7 Mekanizmalar\u0131<\/figcaption><\/figure>\n\n\n\n<h3>Adrese Teslim Antibiyotikler Nano teknoloji ile M\u00fcmk\u00fcn m\u00fc?<\/h3>\n\n\n\n<p>Ku\u015fkusuz ki diren\u00e7 geli\u015fiminin en b\u00fcy\u00fck nedeni a\u015f\u0131r\u0131 dozda ila\u00e7 kullan\u0131m\u0131d\u0131r. \u0130lac\u0131n v\u00fccudun her yerine da\u011f\u0131lmas\u0131 yerine enfekte b\u00f6lgede birikmesini sa\u011flamak, ilac\u0131n biyo yararlan\u0131m\u0131n\u0131 art\u0131r\u0131r. Yani k\u00fc\u00e7\u00fck miktarla dahi b\u00fcy\u00fck bir etki elde edilir. Ara\u015ft\u0131rmac\u0131lar bunu nano materyaller arac\u0131l\u0131\u011f\u0131yla m\u00fcmk\u00fcn k\u0131ld\u0131. \u00d6rne\u011fin bir \u00e7al\u0131\u015fmada bakterilerin kolayl\u0131kla diren\u00e7 geli\u015ftirdi\u011fi ve \u00e7ok\u00e7a yan etkisi bulunan bir antibiyotik olan triklosan, nano materyaller sayesinde pozitif y\u00fckle y\u00fcklendi, negatif y\u00fckl\u00fc bir h\u00fccre duvar\u0131na sahip olan bakteri ile daha kolay etkile\u015fime girmesi sa\u011fland\u0131 ve ila\u00e7 o b\u00f6lgede daha \u00e7ok birikti. B\u00f6ylece ayn\u0131 etki i\u00e7in daha k\u00fc\u00e7\u00fck dozajlar yeterli oldu ve hem ilac\u0131n yan etki ihtimali hem de bakterilerin diren\u00e7 geli\u015ftirme olas\u0131l\u0131\u011f\u0131 azalt\u0131ld\u0131. <\/p>\n\n\n\n<p>Benzer bir y\u00f6ntem mide \u00fclserlerinin \u00f6nemli bir nedeni olan Helicobacter Pylori bakterileri i\u00e7in kullan\u0131ld\u0131. Antibiyotik i\u00e7eren nano partik\u00fcller, H. Pylori nin ba\u011flanabilece\u011fi antijenler i\u00e7eren mide epitel h\u00fccrelerinin membranlar\u0131yla kapland\u0131. \u00a0\u0130n vitro ortamda bu partik\u00fcllerin bakterilere daha \u00e7ok yap\u0131\u015ft\u0131\u011f\u0131 ve partik\u00fcl i\u00e7indeki ilac\u0131n, serbest ilaca oranla \u00e7ok daha etkili oldu\u011fu g\u00f6zlemlendi. <\/p>\n\n\n\n<p>Enfekte b\u00f6lgenin olu\u015fturdu\u011fu mikro \u00e7evre de antibiyotiklerin se\u00e7ici iletimine yard\u0131mc\u0131 olur. Enfekte b\u00f6lgede oksijen yetersizli\u011fi nedeniyle anaerobik glikoliz bask\u0131nd\u0131r ve sonu\u00e7 olarak b\u00f6lgede pH azal\u0131r. Bu asidite pH\u2019a duyarl\u0131 nano ta\u015f\u0131y\u0131c\u0131lar\u0131n geli\u015ftirilmesiyle antibiyotiklerin sadece gerekli b\u00f6lgelere iletilmesine yard\u0131mc\u0131 olur. <\/p>\n\n\n\n<h3>Bakteriler Art\u0131k Makrofaj \u0130\u00e7inde Saklanamayacak <\/h3>\n\n\n\n<p>Makrofajlar\u0131n bakterileri yutmaktaki amac\u0131 onlar\u0131 sindirmek olsa da bakteriler ka\u00e7ma mekanizmalar\u0131yla bundan kurtulabilir, makrofaj i\u00e7inde latent kalarak daha sonra hastal\u0131\u011f\u0131n tekrar\u0131na yol a\u00e7abilir ve makrofajlar\u0131n aktif hareketiyle v\u00fccudun farkl\u0131 b\u00f6lgelerine ta\u015f\u0131nabilir. Ayr\u0131ca antibiyotiklerin makrofajlarda kalan bakterilere ula\u015fmas\u0131 ve onlar\u0131 \u00f6ld\u00fcrmesi enfeksiyon b\u00f6lgelerindekilere g\u00f6re daha zordur. Bu zorlu\u011fu a\u015fmak i\u00e7in makrofajlar\u0131 ve h\u00fccre i\u00e7i patojenleri hedefleyen nano partik\u00fcller geli\u015ftirilmi\u015ftir. Partik\u00fcller i\u00e7ine y\u00fcklenen antibiyotik ila\u00e7 bakteriye ula\u015ft\u0131\u011f\u0131nda bakterinin \u00fcretti\u011fi fosfolipaz vb. enzimlerin aktivitesi sonucu nano partik\u00fcllerin y\u0131k\u0131lmas\u0131yla a\u00e7\u0131\u011fa \u00e7\u0131kar ve etkisini g\u00f6sterir. <\/p>\n\n\n\n<h3>Biyofilm Olsa da Ka\u00e7\u0131\u015f Yok<\/h3>\n\n\n\n<p>Biyofilme sahip bakteriler i\u00e7in temelde iki strateji kullan\u0131l\u0131r. Birinde biyofilm mikro \u00e7evresinin sahip oldu\u011fu \u00f6zellikler kullan\u0131larak nano ta\u015f\u0131y\u0131c\u0131lar \u00fcretilebilir. Di\u011fer strateji ise bakterisidal aktiviteyi art\u0131rmak i\u00e7in var olan mikro \u00e7evrenin de\u011fi\u015ftirilmesidir. \u00d6zellikle biyofilm \u00e7evresinde hakim olan hipoksinin hafifletilmesi ile seftazidim, piperasilin ve tazobaktam gibi antibiyotiklerin biyofilme kar\u015f\u0131 olan etkinliklerinin artt\u0131\u011f\u0131 vurgulanm\u0131\u015ft\u0131r. <\/p>\n\n\n\n<h3>\u0130norganik Nanomateryaller<\/h3>\n\n\n\n<p>G\u00fcm\u00fc\u015f, alt\u0131n, ZnO, MoS2, Fe3O4 ve benzeri metal partik\u00fcller genelde reaktif oksijen \u00fcr\u00fcnleri \u00fcreterek; h\u00fccre duvar\u0131 y\u0131k\u0131m\u0131, metabolik s\u00fcre\u00e7lere m\u00fcdahil olma, enzim inaktivasyonu, DNA hasar\u0131  gibi pek \u00e7ok mekanizmayla bakterilere zarar verebilir. Ancak bu maddeler memeli h\u00fccreleri i\u00e7in de toksiktirler. Bu y\u00fczden kullan\u0131mlar\u0131 k\u0131s\u0131tl\u0131d\u0131r. \u00d6zellikle g\u00fcm\u00fc\u015f bu anlamda en \u00e7ok kullan\u0131lan metaldir. Alt\u0131n\u0131n tek ba\u015f\u0131na bakterilere etki etmedi\u011fi bilinse de baz\u0131 bakterisidal ajanlar\u0131n alt\u0131n nano partik\u00fclleri varl\u0131\u011f\u0131nda etkinliklerinin artt\u0131\u011f\u0131 bildirilmi\u015ftir. <\/p>\n\n\n\n<h3>Nano Malzemelerin Kullan\u0131m\u0131nda K\u0131s\u0131tlamalar<\/h3>\n\n\n\n<p>Nano ila\u00e7lar\u0131n kompleks fizyolojik \u00e7evrede stabilitesini koruyabilmesine y\u00f6nelik endi\u015feler vard\u0131r. Asl\u0131nda kanser tedavisi i\u00e7in fare modellerinde yap\u0131lan in vivo deneyler bu endi\u015feyi g\u00fc\u00e7lendiriyor. Polimerik nano antibiyotikler, Poli Etilen Glikol veya di\u011fer molek\u00fcllerin modifikasyonu ile daha y\u00fcksek stabilite g\u00f6sterebilir. Ancak lipozomlara k\u0131yasla, bu maddelerin d\u00fc\u015f\u00fck ila\u00e7 y\u00fckleme h\u0131zlar\u0131 ve tart\u0131\u015fmal\u0131 biyo uyumluluklar\u0131, klinik uygulamalar\u0131n\u0131 engellemektedir. <\/p>\n\n\n\n<p>Ayr\u0131ca nano ila\u00e7larla yap\u0131lm\u0131\u015f klinik deneyler olduk\u00e7a\nazd\u0131r. Bu nedenle uzun vadedeki ak\u0131betleri konusunda net bir bilgi yoktur.\nH\u00fccresel metabolizmay\u0131 nas\u0131l etkiledi\u011fi de ayd\u0131nlat\u0131lamam\u0131\u015f konulardand\u0131r. <\/p>\n\n\n\n<p>\u00d6zellikle metal nano partik\u00fcller v\u00fccutta toksisite yaratabilir.
